Paperwork Instructions:
New patient paperwork must be completed and
returned to the office at least 1 week prior to your first appointment.
Please complete the appropriate Information Packet; the Signature Page for Consent and Agreement; and the Consent to Release Protected Health Information, if needed. To return paperwork you may send it via secure email to info@mtmkc.com or drop it off to the office at at scheduled time.

Important Notice
During these winter months, if the Blue Valley School District is closed due to inclement weather, the clinic will be closed as well to ensure everyone’s safety. If there is a late start due to weather, the clinic will also have a late start. All messages and rescheduling of appointments will be addressed as quickly as possible once we return to the clinic.
